Please be informed that TM made some changes on documentation policy to TM Unifi & Streamyx Business (Effective 1st October 2013):

Director IC must be provided.
a) For Sdn Bhd, director IC’s name must be matched with the name stated in Form 49.
b) For Sole proprietor, besides SSM form, you must provide additional form where director’s name stated.

For those orders which is handle by the person in-charge, you must provide an authorization letter for the applicant. Items below must be included in the authorization letter:
a) Name of the applicant
b) Copy of IC of applicant
c) Address for the installation
d) Package
e) Name of agent and principal name
f) NIRC of agent
g) Applicant and agent signature with date and time

However, the director IC must be attached during registration.
